Release of Liability
Parents of sophomores will be notified in December regarding their daughter's ACTS placement. Included in the mailing will be a Release of Liability and Emergency Contact form that must be signed by the parent in order for their daughter to participate in the ACTS program. NOTE: If your daughter's assigned agency cannot accomodate our students on the Thursday of Mission Week, there will be an additional Release of Liability form for you to sign, allowing her to do service at one of our other agencies for that day only.
Should your daughter's agency have additional paperwork relating to her service, you will receive it in this same mailing.
It's very important that all paperwork is completed and returned to the ACTS office no later than December 11.

ACTS Fee
To cover the cost of program activities, materials, and transportation, the business office will assess each incoming sophomore a $60 fee, payable by July 1.
ACTS Uniform
To meet the dress requirements of various agencies and to project a positive image while performing service in the commmunity, all sophomores are required to wear black, AHS monogrammed Dickies uniform pants (available at Shaheen's) with a uniform blouse/polo on ACTS Wednesdays (and Thursday of Mission Week) during the second semester. School sweaters may also be worn, but sweatshirts may not. Please allow ample time to have the Dickies monogrammed with the school logo. Students have been advised to place their order no later than the end of October.

TB Skin Test
All sophomore students must have a TB skin test taken in order to perform service at one of our ACTS agencies next spring. This certificate is separate from the immunization records requested by Assumption's main office and must be dated June 1, 2009, or later.
- Parents may choose to have the TB skin test administered by their own private physician or through a clinic. The original (usually pink) certificate showing that the test has been given and read must be turned into the ACTS office by NOVEMBER 1, 2009.
- If a student has tested positive in the past as a result of a TB skin test, they will need to supply the ACTS office with documentation stating this and detailing any subsequent treatment.
- The Health Department will provide in-house testing on September 28 and September 29 during your daughter's Theology class. The cost for this service is $20, payable to Assumption High School. A detailed letter and permission form will be sent to parents in mid-September.
