| ACTS Requirements | |
TB Skin Test
All sophomore students must have a TB skin test taken in order to perform service at one of our ACTS agencies next spring. This certificate is separate from the immunization records requested by Assumption's main office and must be dated June 1, 2007, or later.
- Parents may choose to have the TB skin test administered by their own private physician or through a clinic. The original (usually pink) certificate showing that the test has been given and read must be turned into the ACTS office by December 7, 2007.
- If a student has tested positive in the past as a result of a TB skin test, they will need to supply the ACTS office with documentation stating this and detailing any subsequent treatment.
ACTS Uniform
To meet the dress requirements of various agencies and to project a positive image while performing service in the community, all sophomores are required to wear black Dickies uniform pants (available at Shaheen's) with a uniform blouse/polo on ACTS Wednesdays* during the second semester. The gray AHS sweater may be worn however, sweatshirts are not consider part of the ACTS uniform. Please allow ample time to have the Dickies monogrammed with the school logo. Students have been advised to place their order no later than the end of October.
*The ACTS uniform will also be worn on Thursday of Mission Week.
ACTS Fee
There is a $55 fee for ACTS to cover the costs of program activities and student transportation. The business office will send a statement reflecting this charge which is due by July 1.
Release of Liability
Parents of sophomores will be notified in December regarding their daughter's placement at an ACTS agency. Included in the letter will be a Release of Liability and Emergency Contact form that must be signed by the parent in order for their daughter to participate in the ACTS Program. NOTE: If your daughter's assigned agency cannot accomodate ACTS students on the Thursday of Mission Week there will be an additional Release of Liability form for you to sign.

| ACTS Assignments | |
The ACTS assignments are completed and will be given out to sophomores during study halls beginning November 26. NOTE: In order to know your placement you must (1) have your planner with you in study hall and (2) have your TB certificate on file in the ACTS office.
